SERVING THE HOMEBOUND
Too often the aged or infirm are unable to obtain needed medical care due to confinement to their residence. Medical care was traditionally provided at the patient’s home. The shift to providing care in an office environment has left some patients without medical care.
Recent technological advances in communications, equipment portability, computerization of medical records and medical diagnostic equipment have allowed our providers to serve the homebound patient with the exact same level of care as the care that which could be provided in an office environment.
WHO DO WE SERVE?
Within our geographical area, we treat eligible patients that lack the mobility to travel to a physician’s office due to disability, infirmity or recent discharge from a hospital. Care is delivered in the patient’s home or at their residence in an assisted care facility.

OUR SERVICES
Sun Medical Care, P.C. provides house call visits during the weekdays by appointments. Currently, we service Schenectady, Albany and Southern Saratoga counties.
We provide services that would normally be performed in an office environment. The provider travels not only with the traditional “doctor’s bag” but also with the latest technological tools to include a computer, EKG, and other diagnostic tools.
Question: I already have a doctor?
Sun Medical Care providers can serve as consultants to your current provider with a referral. If you do not currently have a doctor we can become your personal provider.
Question: What if I need to be hospitalized?
Drs. Sun and Welch have affiliations with Ellis and St. Clare’s Hospitals. If you are admitted to other hospitals, arrangements would be made for other hospital physicians to provide care. Upon discharge, we will coordinate ongoing care between your home health agency, our medical group, and other caregivers. |
